Position Overview: Clinical Pharmacist
As a Clinical Pharmacist, you will play a vital role in improving patient outcomes by delivering clinical pharmacy services designed to optimize medication therapy and support healthcare goals. Your expertise will be essential in managing a broad range of Medication Therapy Management (MTM) services, including Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring, Cost-Effective Solutions, and Transitions of Care. This role focuses on providing personalized care to patients, helping them manage and improve their medication regimens.
Key Responsibilities:
Provide MTM services, such as Comprehensive Medication Reviews, Disease-Specific Medication Reviews, and Targeted Interventions to evaluate medication appropriateness, identify adherence barriers, optimize therapy, and enhance clinical outcomes.
Review patients' medication profiles, applying evidence-based guidelines to recommend improvements in their medication regimens and collaborating with healthcare providers.
Assist in managing medication costs by recommending cost-effective solutions, including formulary or therapeutic changes, pill splitting, combination products, and eliminating unnecessary or duplicate medications.
Thoroughly assess prescribed therapies for issues like unnecessary medications, gaps in patient education, ineffective treatments, dosage concerns, side effects, and adherence challenges.
Create and deliver Personal Medication Lists (PMLs) through comprehensive medication reconciliation, along with Medication Action Plans (MAPs) tailored to each patient's needs. Communicate clinical recommendations and interventions to prescribers and interdisciplinary teams.
Advocate for MTM services and clearly explain their benefits to patients, families, and healthcare providers, ensuring they understand how these services contribute to better health outcomes.
Maintain a high standard of customer service by responding promptly, professionally, and courteously to all patients and providers.
Follow up with patients and providers to monitor progress towards treatment goals and make adjustments as needed.
Serve as a trusted drug information resource for patients, healthcare providers, and the broader care team.
Provide additional clinical pharmacy services as requested by the manager.
Meet productivity and performance expectations outlined by management.
Qualifications & Requirements:
A reliable high-speed internet connection (cable broadband or fiber optic service provider).
A Bachelor's degree in Pharmacy or Doctor of Pharmacy (Pharm D.) from an accredited pharmacy school.
At least 2 years of experience as a clinical pharmacist.
Experience in Medication Therapy Management is preferred.
Current, unrestricted pharmacist licensure in your state.
Willingness to reciprocate your pharmacist license to other states as needed.
Strong interpersonal and customer service skills, with the ability to communicate effectively with diverse audiences.
Excellent written and verbal communication skills.
Proficiency with technology, including MS Office (especially Excel and Word) and Google G-Suite.
Strong IT skills, including troubleshooting experience.
Physical Demands:
The role involves occasional sitting, standing, walking, bending, and stooping.
Standard office activities, such as using a computer, typing, and speaking on the phone, may be required for extended periods.
The work environment is office-based and follows HIPAA, ARRA/HITECH, and other regulatory standards to maintain confidentiality and security.
This position is based in a home office setting, providing flexibility for remote work.
