Job Title: Registered Nurse (RN) Home Health

Shift Times: 8hour days MondayFriday
Salary Range: $36.42$60.33 per hour (based on experience and education)
Job Description:
The Home Health RN is responsible for assessing patient physical psychological social spiritual educational developmental cultural and discharge planning needs. The role involves reviewing patient history and physical with patient/family interpreting diagnostics and laboratory data and formulating a comprehensive plan of care. The RN works closely with other healthcare team members to ensure optimal patient care and effective discharge planning.
