This Social Work Care Manager Position Features:
• Make a huge impact the aging & elderly community of the South Bay
• Work primarily Monday to Friday, will just occasional weekend events
• Great Pay of $66k o $70K

Immediate need for a local non-profit organization that services the need of aging community & veteran community. This immediate need as a Social Work Care Manager.

You will be responsible for offering client-centered case management to older adults, veterans, and adults with disabilities. The primary goal is to facilitate safe aging in place, reduce emergency room visits, and prevent institutionalization by implementing person-centered planning and coordinating care. The Care Manager will function as a part of the Client Information & Awareness Team providing services throughout all of the South Bay to administrate a program which will assist older adults to remain in their homes significantly decreasing costs to health care insurers, families and public agencies, by providing Information & Referral, short-term service coordination, benefit enrollment assistance, and case management to vulnerable older adult populations within South Bay Area.

EDUCATION & EXPERIENCE:
• • Bachelor’s degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology or related field.
• • At least 2 Years of Case Management experience including care planning.
• • At least 2 years of experience working with older adults and/or vulnerable populations.
• • Effective oral and written communication skills
• • Valid California Driver’s License
